How do I request an absentee ballot?

Where do I send the request?
You will need to mail (not fax) the completed form to your county clerk. A list of county clerks is provided with the absentee ballot application. You can also find their contact information here. (If you follow this link, be sure that you mail your request to the county clerk and NOT to the superintendent or the board of elections.)
What is the deadline for requesting an absentee ballot?
You may request an absentee ballot by mail until seven days prior to Election Day:
October 28 for the General Election (by mail). It is advised, however, that you submit your application several weeks before Election Day in order to allow plenty of time for items to travel through the mail. You may also request an absentee ballot in person at the county clerk’s office until 3 PM on the day before Election Day.
What is the voting procedure?
Simply fill out your ballot, sign it, and return it. No witnesses are required.
What are the deadlines for voting absentee?
Your absentee ballot must be received by the County Board of Elections no later than the close of polls on Election Day:
General Election: November 4

General Election: November 4
Voter registration closes 21 days prior to Election Day.
